Take a few minutes to enroll your child in our Home Visiting Program.
The Select Home Visitation program is a voluntary program made up of Parent Educators that develop educational planning guides to address the family’s needs. The Parent Educators use an evidence-based early childhood home visitation model called Parents as Teachers (PAT). Parents as Teachers is an approved home visiting model meeting the evidence-based criteria of the Maternal, Infant, and Early Childhood Home Visiting program. Parent Educators emphasize parent-child interaction, development-centered parenting, and family well-being as they deliver services to families. The PAT curriculum is designed to increase parents’ knowledge of their child’s health and development, school readiness and need for parent involvement.

Select Home Visitation Program
First5LA
Qualification
Enrollment is through via referral from First 5 LA’s Welcome Baby Program that utilizes a screening tool to assess families at birth for services. Families residing within a First 5 LA Best Start Community (East Los Angeles and South East Los Angeles) are eligible for referral to Plaza Community Services, Select Home Visitation program. The hospitals from which we are receiving referrals from are White Memorial Medical Center and St. Francis Medical Center.

Qualification
Enrollment is via referral from the Department of Public Health, or self-referred. Participants that reside in SPA 4 are eligible to participate in the program. Participants must meet program requirements. Participants must be pregnant or have a child younger than 36 months.

Qualification
The target population are first-time parenting teens with a child 24 months or younger. The CalWORKs (first-time parent) participant must have a child 24 months or younger at the time of enrollment only. Participants are enrolled into via referral through the CalWORKs program.
